Since the iPhone 5s, every iPhone has shipped with a motion co-processor (the "M-series" chip) connected to an accelerometer and gyroscope. It runs continuously at extremely low power and recognises the rhythmic pattern of walking. That means step detection happens at the hardware level — an app doesn't need GPS, background refresh tricks, or a wearable to know you walked.
What a watch adds — and what it doesn't
A watch helps if you often walk without your phone, or if you want wrist-based heart rate. For pure step counting, studies comparing phone and wrist trackers consistently find phones are similarly accurate for normal walking, because your phone moves with your body. If your phone is usually with you — pocket, bag, hand — you're already carrying a pedometer.
